The Evolution of Guided Surgery: From Traditional Implants to Zygomatic and Pterygoid Implants
The evolution of guided surgery has significantly transformed implant dentistry, expanding treatment possibilities from conventional implant placement to advanced solutions for severely atrophic maxillae. What began as prosthetically driven planning for traditional axial implants has progressed into highly sophisticated digital workflows that now support the precise placement of zygomatic and pterygoid implants.
This lecture explores the clinical and technological evolution of guided surgery, highlighting the transition from static guides for standard implants to advanced digital protocols capable of managing complex anatomical challenges. Emphasis will be placed on digital planning, CBCT integration, prosthetically driven workflows, and the biomechanical considerations required for long implants in remote anchorage sites such as the zygoma and pterygoid region.
Through clinical cases and step-by-step workflow analysis, participants will gain insight into how modern guided solutions enhance surgical accuracy, reduce invasiveness, improve safety, and enable immediate loading protocols - even in extreme atrophy cases. The session will also discuss limitations, risk management, and the future direction of fully digital full-arch rehabilitation.
Attendees will leave with a comprehensive understanding of how guided surgery has evolved from conventional implant placement to advanced zygomatic and pterygoid applications, redefining the boundaries of full-arch rehabilitation.

Implant and Abutment Selection for Long-Term Success
Long-term implant success is less about “the best implant” and more about selecting a biologically coherent implant–abutment–prosthetic system that preserves the supracrestal tissue seal under real-world mechanical loading and patient-specific risk.
Current evidence increasingly suggests that peri-implant complications arise from a soft-tissue–initiated inflammatory process whose intensity and progression are strongly modulated by structural context(absence of periodontal ligament), micro-mobility, and material-associated stimuli, rather than by “unique” peri-implant pathogens alone.
The aim of this lecture is to provide a clinically actionable framework for implant and abutment selection that prioritizes mucosal stability, prosthetic-driven biomechanics, and risk-adapted maintenance, with explicit decision points that translate biology into daily treatment planning.
This presentation synthesizes the mechanistic cascade from (1) implant macro-design (emergence profile, platform management, tissue thickness), to (2) implant–abutment interface behavior (microgap, micro-movement, connection stability), to (3) prosthetic architecture (contour, restoration type, occlusal scheme), and (4) material choices (titanium vs zirconia transmucosal components) in relation to plaque retention, tissue inflammation, and long-term complication profiles. Emphasis is placed on predicting complexity preoperatively, matching cases to operator skill, and communicating the biological rationale to patients.

Thinking Outside the Box in Soft Tissue Management
Current hot topics in reconstructive dental surgery include papilla reconstruction, supracrestal attachment regeneration, and the management of peri-implantitis in the esthetic zone.
At the same time, patients increasingly seek treatment approaches that minimize the number of surgical interventions. Traditional methods—such as removing the implant-supported crowns, placing cover screws, allowing the tissue to heal over them, performing subsequent soft- and hard-tissue reconstruction, and uncovering the implants—require multiple surgeries and significantly prolong overall treatment time. This leads to a fundamental question: How can we meet our patients’ expectations for predictable outcomes with only a single surgical procedure?
This lecture will present innovative concepts designed to address these demanding clinical situations. It will focus on how papillae and supracrestal attachments can be predictably reconstructed and which new strategies are necessary to achieve these goals.

Biological Width and Biological Space: Why Does It Matter on Natural Tooth and Convergent Neck Implants
The biological width represents the functional seal of soft tissues (junctional epithelium and connective attachment) essential to protect the alveolar bone from bacterial aggressions of the oral cavity.
Although its protective function is universally known on natural tooth, its clinical configuration and response vary significantly among different implants design and platform.
While on the natural tooth the biological space is a defined anatomical entity to be preserved, in the modern implants is an entity to be planned.
The adoption of an implant with a convergent neck geometries allows the strategic management of peri-implant tissues, guaranteeing a long-term aesthetic and functional integration superior to classical bone level profiles.

Rehabilitation Strategies for Atrophic Maxilla
Atrophic maxila challanges the clinician with irreversible bone loss due to tooth extractions, sinus pneumatization, periodontal disease, resulting in inadequate vertical/horizontal bone volume and poor density, limited space for dental implants, increased interarch space, and difficult prosthetic solutions. The patients require more and more graftless implant techniques to overcome severe bone deficiencies and achieve long-term functional: pterygoid , zygomatic , nasal or tilted implants are developed for this purpose. In any atrophic case, prosthetic-driven implant planning can place screw-retained restaurations in a more esthetic and functional position reducing the cantilever, the screw overload and failure. The alveolar position of the implant is dictated by the position of the future abutment. Remote ancorage ofers primary stabilitiy and the steep angulation is corrected knowing the multiunit abutment challanges: the more angulated the multiunit - the more distal is the insertion of the abutment screw- the prosthodontic final result is different. Knowing the local anatomy, finding the cortical areas for bi- or tri-cortical fixation of the implant, as well as precisely planning of the prosthodontic, eliminate all guesswork from the process, reduce the complications and increase patient satisfaction.

New Perspectives on the Microbiome: Oral Probiotics, Systemic Connections
Research on the human microbiome in recent decades has opened up new perspectives on understanding the relationship between oral and systemic health. This presentation will review the
importance of oral microbiome balance, the scientific basis for the use of oral probiotics, and the potential role of beneficial bacteria in supporting biofilm ecology. Currently known and hypothesized connections within the gut-oral axis will be discussed. The presentation will conclude with an
overview of the development concept behind Proboil DUO, an innovative probiotic formulation designed for oral use that aims to support the balance of the oral microbiome.
