The concept of canal preparation with only one mechanical instrument used in reciprocation was introduced several years ago. Studies and clinical research have shown the efficiency and the safety of the Reciproc® instrument in the preparation of the majority of canals without creating a glide path, and in the retreatment procedure. Reciproc® blue, a thermally treated instrument, is the new generation of the original Reciproc® instrument; it has an increased resistance to cyclic fatigue and a greater flexibility enabling a safer and smoother canal preparation procedure, and a wider range of clinical applications. Reciproc® blue will be introduced in this presentation. The physical properties of this new instrument will be discussed with emphasis on their impact on the clinical applications. Preparation of the majority of the canals including 2nd mesio-buccal canals in maxillary molars, and narrow and curved canals with only one Reciproc® blue instrument without an initial canal negotiation and without a glide path will be described. The need to create a glide path in some canals with a reciprocating glide path instrument, R-Pilot®, will also be discussed.

„Nil nocere” should be a must during our endodontic treatments. Often we work routinely, applying the same protocol for every case. For example by retreatments, we usually get used to follow the „all or none” approach – removing all restorative and obturation materials from every root canal – regardless of the presence or absence of periapical pathosis. In my opinion this approach should not always be followed. During my presentation, i will demonstrate step by step protocols to carry proper selecive endodontic retreatments. Using the guided approach, retreating just the root or roots, which show a clear signs of pathosis. With the help of conservative approach, using magnification, CBCT, and ultra-flexible instruments, static or dynamic navigation, guided selective retreatment is a considerable option. The main goal should always be to ensure a functional root canal treated tooth for a long period of time for our patients. That’s why an individualised treatment plan and protocol is necessary, to be less invasive as reasonable, to save as much sound structures as possible.

In the last 20 years endodontics has been closely linked to science and
technology, and it is precisely this connection that has allowed the specialty to have a steady growth becoming today one of the most
advanced medical disciplines in existence. However, knowing more has made us understand that we still have a long way to go when it comes to ensuring a predictable therapy for our patients and understanding the pathology of the pulp. Specialists preoccupy themselves more and more with the creation of safe protocols that may ensure endodontic success on a larger scale. Most of the dentists are unfortunately unaware of the big changes that have happened in endodontics and thus rarely take advantage of the wonderful new technologies that one has at his disposal. I cannot stress enough about how understanding the new materials and instruments may better help the dentist in reaching the goals of an endodontic treatment. During my lecture I will discuss in detail the philosophy of modern day endodontics and about how the new files are specifically designed, with great consideration for everything that evidence based dentistry has taught us. Predictable endodontic treatments are a reality, let’s start preforming them today!

Lecture will provide cases of root canal treatment with novel approach to hard tissues. During the lecture there will be an analysis of all possibilities and threats of these procedures.

Root canal treatment has been codified since the mid-1900s and aims at eliminating inflamed and/or infected tissues from the root canal space after debridement and shaping is performed and seal this space to maintain the treated tooth as a functional unit. The evolution of devices and instruments has followed and even reached frightening extents. With the advent of engine-driven systems, optical enhancement, 3D visualization devices, ultrasonic technology and new filling
materials, operators become so focused on the technical aspect of the treatment that they often lose perspective of the bigger picture, thus remaining trapped in the limitations of treatment concepts and fighting to gain the advantages of technological advances. Although these advances are not absolute, they could improve on our performances and help us catch the wave of minimally invasive dentistry in
Endodontics… but do we need them all?
