Dr. Melkers is a 1994 graduate of Marquette University School of Dentistry & currently serves as Visiting Faculty at Rīga Stradiņš University in Riga, Latvia & as Advisor and Visiting Faculty for The Pankey Institute in Key Biscayne, Florida and Lead Educator for RIPEGlobal.

Dr. Melkers is a Fellow & Master of the Academy of General Dentistry and holds additional Fellowships in the International Congress of Oral Implantologists and the American College of Dentists. He maintains a private clinical practice in Hanover, NH with a focus on comprehensive adult restorative care. Dr. Melkers balances his professional time between clinical practice, teaching, publishing and research.

This lecture presents a concise overview of advanced strategies in the additive rehabilitation of tooth wear, emphasizing the judicious use of direct and indirect restoration techniques alongside the integration of digital technologies. The treatment protocol initiates with the conservative restoration of the lower and upper frontal segments.

Through this approach, we not only restore the worn frontal teeth, enhancing aesthetics and function, but also elevate the vertical dimension of occlusion (VDO). This elevation is crucial for creating sufficient space to restore the posterior segments while simultaneously ensuring the mandible's central position is preserved. The lecture delves into various composite restoration techniques for both frontal and distal teeth segments, transitioning into a discussion on the seamless integration of direct and indirect methods.

Furthermore, we explore the role of digital protocols in enhancing the predictability and functionality of the outcomes. This comprehensive approach not only preserves the existing healthy tooth structures but also aims at achieving functional and aesthetic rehabilitation of the masticatory apparatus, illustrating the process through documented real-life clinical cases.

Born in Sofia in 1974. Graduated from the Faculty of Dental Medicine Sofia in 1998.
Assistant professor at the Department of Conservative Dental Medicine, FDM-Sofia from 1999 until 2010.
He acquired a postgraduate specialization in "Operative dentistry and endodontic" in 2003.
At present Dr. Raychev runs his own dental practice, and is a part-time assistant professor at the Department of Conservative Dental Medicine. His current practice is devoted to the concept of realization the optimal esthetic results in the least invasive, safe and consistent treatment mode. Dr. Raychev is author and coauthor of publications in scientific journals, lecturer and invited speaker in national and international congresses, KOL for Ivoclar.

Introduction to Clear Anatomical Matrixing Systems and Injection Moulding Composite Techniques

Many dental practitioners have trouble with staining, layering, chipping and overhangs in anterior and posterior composite restorations. The Bioclear Method is a technique utilizing clear anatomical shaped matrices to create strong beautiful, tooth-shaped, stain resistant restorations. It is a specific technique with multiple steps along with new armamentarium that can transform how one approaches composite restorations.

The closing of black triangles, a cosmetic issue that with traditional composite techniques most clinicians find difficult, or impossible, to do. While there is a learning curve and a specific sequence that should be followed, this method is doable by any clinician, not just those with golden hands. The purpose of this lecture is to introduce you to what the Bioclear Method is and all the steps involved in getting started.

Dr. Jarett Hulse graduated from the University of Pittsburgh, in the United States, in 2005. He did a one-year General Practice Residency in New York City. He has completed approximately 2000 hours of continuing education and received his Masters in the Academy of General Dentistry in 2016. He’s owned his dental practice, Austin Artistic Dental, since 2011 in Austin, Texas. What started out as a family general practice has evolved into a largely Cosmetic and Complex Restorative practice. His Bioclear journey began in 2014. He has taken all of Bioclear’s available courses, and has been a Bioclear Key Opinion Leader for the past 6 years. This includes running some Bioclear lectures.

Dr. Hulse largely credits Bioclear for helping bring his clinical skills to the next level, which allowed him to offer a level of service that many other dentists could not. Dr. Hulse is also a founding member of Practice on Fire. A business and clinical mastermind group that started on Dentaltown.com in 2014. The marketing and communication skills he helped develop in Practice on Fire, along with his clinical skills, has helped grow his practice. Now he is largely doing the type of dentistry that interests and excites him.

Direct vs Indirect Restorations in Esthetic Area: Clinical Protocols

The ever-increasing demand of esthetic results and restorative treatments from patients forces the clinician to provide a perfect appearance outcome and to be owner of different types of treatments and approaches. The indications about indirect restorations or direct restorations are becoming ever less absolute: considering many clinical aspects you can opt for a protocol rather than the other. The versatility of the composite resin, its possibility of layering intraorally, the ease of repair and the fairly good prognosis in the medium and long period, making it a reliable material and the tendency of new materials to improve the wear resistance and the stability of the surface candidates it in many cases as a worthy substitute of ceramic materials.

By contrast, ceramic materials, often considered the gold standard for colour characteristics and mechanical and physical properties, see an increase in the ability to produce restorations with minimum thicknesses while maintaining dimensional stability, prognosis and aesthetics. The choice, therefore, to adopt a direct or indirect approach is always less obvious, but, on the contrary, offers to dentists greater possibility of diversification in restorative. Finally, many aspects could be discussed as: indications, detailed clinical protocols, communication protocols, esthetic appearance, predictability and durability.

After graduating in Serbia in 1996, Dr Ristic searched the new frontiers emerging trends in esthetic dentistry and implantology. In 1999, he finished specialization in dental prosthodontics with special accent on all-ceramic adhesive restorations. In 2001 established in Belgrade, Serbia, his private practice Center for Dental Esthetic and Implantology. Dr Ristic loves teaching treatment planing and various clinical procedures through lectures, hands-on trainings and workshops with focus on all-ceramic and implant restorative procedures. He gave more than 60 lectures nationally and internationally.

In 2008 Dr Ristic was awarded with a Fellowship from the ‘International Academy of Dental Facial Aesthetics’ in New York, an recognition shared by worlds eminent dental professionals. One of the founding members of Serbian Academy of Esthetic dentistry, Certified ESCD Member of the board, member of BaSS, ITI, AACD and affiliate member of IADFE.

Prof. Louis Hardan is the Head of the Restorative and aesthetic Department at Saint Joseph University Beirut-Lebanon. He graduated dentistry in 1989 and continued his post-doctoral education at Saint Joseph University. Prof Hardan obtained a certificate in basic science in 1993 and completed his specialization in restorative and aesthetic dentistry in 1995, in 2009 completed his Ph.D. in oral biology and materials. Prof Hardan was an active member of the Lebanese Dental Association (LDA) (2006-2009) and was assigned as a General Secretary of the LDA in 2009. At the present, he is an Associate Professor at Saint Joseph University and owns a private practice in his hometown Byblos.

He is an honorary and active international Styleitaliano member, head of the networking and media section and coordinator of all Styleitaliano departments. Additionally, he is the inventor of Smile Lite MDP (Smile Line, Switzerland) a device to take high-quality dental pictures with a mobile phone and Posterior Misura (LM instrument) an instrument for direct posterior composites. Prof. Hardan has many publications in international journals and has given several lectures on aesthetic, restorative dentistry and mobile dental photography.

Today more and more patients seek aesthetic dental treatments. As dental professionals, we should be able to treat them in order to restore not only their esthetics, but also their biology, structure and function, giving them a natural restoration of their smile. For that to be accomplished, an extensive documentation with photos and videos and a careful treatment plan is fundamental before starting the treatment. We need to consider several parameters, such as morphological characteristics of each individual tooth (anterior tooth basic anatomy), their alignment and position in the arch, their color and intra-oral criteria that define an esthetic smile composition, such as: midline line, smile line, incisors proportion and dominance, axial inclination of anterior teeth, vestibular axial inclination of posterior teeth, size of the contact areas, cervical and incisal embrasures, and gingival architecture.

Worn Dentition? Direct and Indirect Adhesive Restorative Approach

The main goal of this lecture is to provide, indications and protocols to diagnose and treat severe worn dentition through a new no prep “hybrid” approach increasing the VDO: 1) a direct workflow “The Index technique”; 2) The lecture will also focus on when and how to bond on worn dentition, additive partial indirect restorations (onlays/overlays) in composite and/or ceramic, depending on the indications. All the step by step protocols will be addressed through step by step images and videos on patients, in order to have a clear over view of this hybrid (direct-indirect) technique through an analog and digital workflow.

She works full time in multidisciplinary dental practice in Tallinn (Estonia) concentrating her work on functional and aesthetic rehabilitation on teeth and implants. Treatment with ceramic veneers is a well documented, widely accepted treatment option today. It can easily be standardized, yet still, many doctors face serious difficulties during these treatments, resulting in sub-standard care and unhappy patients. With ethical attitude and humility towards the profession, respecting some easy steps could ensure avoidance of these failures. In my lecture, I will share some of my thoughts about this topic.

In the 18 years since I graduated as a dentist, I have achieved a level of aesthetic dental treatment in our country that meets the highest international standards, and was previously available in very few places. In addition to the professional quality of my treatments, I pay special attention to the communication with my patients. My aim is also to pass on the knowledge I have acquired, and this knowledge has now been made available to several generations through my lectures, courses and book chapters.

In 2004 Dr. Pawel Szuba-Paszkiewicz graduated with distinction from the Faculty of Dentistry at the Medical University in Wroclaw. As a student he started to obtain his professional experience in a dental practice run by his parents, which enabled him to gain invaluable practical knowledge and skills. The skills in question, turned out to be necessary and extremely motivating while dealing with difficult clinical cases. Since 2002 Dr. Paszkiewicz has participated in many professional courses, conferences and symposia which subjects were related to the latest trends in endodontics, prosthodontics and digital dentistry. As far as professional development is concerned, his skills also developed in collaboration with the prestigious dental clinics in Wroclaw (Poland). Currently, Paul Szuba-Paszkiewicz with passion and precision carries out treatments of complex medical procedures in the field of CAD/CAM prosthetics, implant-prosthetics, full-mouth rehabilitations, and minimal-invasive aesthetic dentistry. A Consultant/Inventor/KOL for Carestream Dental, Dexis, Shining3D, DMG, Harvard Dental, Trate Implants, Imes/Icore, 3D Medical Systems, Cerkamed, Tokuyama Dental, Kuraray/Noritake and ModJaw.

In a cooperation with OrangeDental and Cerkamed he conducts research on the use of photoactive disinfection (PAD) in the treatment of periodontal / periapical diseases. Nowadays his professional interest are focused on Digital Prosthetics and implantprosthetics with its complete digital workflow. He has put forward and implemented his own innovatory procedure of tissue retraction during digital prosthoetic procedures (DiaC-Traction – Diatherm-Guided Chemo-Mechanical Retraction), Endo-Resto Single Visit Chairside Restoring Workflow, as well as innovative fast and complex Smile Design&Delivering Protocol (SDDP), based on author’s „Photo-DFA” workflow in collaboration with TastyTech Canada (Smile Designer Pro manufacturer). He has invented an AllonX non-impression protocol leading to final restorations post immediate implant full-arch loading, incorporated with desktop scanner (MEDIT) and ModJaw capturing of natural patient’s functional and mastication movements. Together with ModJaw functional registration, he has created a non invasive protocol of hybrid restorations consisted of milled buccal veneers, combined with direct labial composite restorations in a protocol of full mouth rehabilitation.
